How to create a custom Create/Append anything module with custom icons for the list

I want to have a Custom version of create/append anything module. I had created some custom components that render differently. I want to add these custom components in the create/append anything module.

I also want to have custom icons for my custom components. How to specify that in the create/append anything module?

The original create-append-anything extension should give you some pointers.

Essentially it is sourcing entries from a list, and displays these as part of a dedicated popup menu.
